- Pathway To Senior Care provides resources and assistance with coordinating senior care. Our goal is to help learn about what is important for you, the care level needed, and direct you to the locations that fit best for your care needs and budget. We do the work for you. With our experience, knowledge, and senior care resources; we provide the following services:
- We will make the phone calls for you.
- We set up tours and tour with you
- We can assist with forms needed for Assisted Living
- Set up TB tests , Dr Visits for forms needed for Assisted Living
- We will connect you with the right person at Assisted Living, Board and Care Homes & Home Care To start a partnership with them once you start their services.
- Assist with safe transition from hospital or skilled care.
- We will continue to be your Senior Living Advocate once Care services are in place.
